The fuel surcharge applies only if the box above is checked and is calculated as a percentage of the agreed hourly trucking rate. It is based on the U.S. Energy Information Administration (EIA) Rocky Mountain (PADD 4) retail on-highway diesel fuel price per gallon and is updated weekly. If the box is unchecked, no fuel surcharge will be applied.

05 Payment terms

Net 30

Payment is due within 30 days from the invoice date.

06 Customer authorization

By signing below, the parties agree to the completed hourly trucking rates, fuel surcharge election and schedule, billing instructions, and Net 30 payment terms in this agreement. The customer representative confirms authority to accept this agreement on behalf of the customer.

07 RS Logistics acceptance

For completion by RS Logistics after the customer submits this agreement.
